Requesting Your Health Care Records
Current and former clients have the right to request copies of their health care records in accordance with applicable Texas and federal law.
To request your records, please submit a written request by:

Requests are processed in accordance with applicable state and federal laws. Any allowable fees will be discussed before records are released.

The Texas Behavioral Health Executive Council investigates complaints involving licensed marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Not every dispute involves professional misconduct, but the Council can provide information regarding the complaint process.
